Introduction: Our vision is that by 2020 Bay of Plenty health services will be centred on the needs of people, their families and Whānau. People will be able to easily access services when required and healthcare workers will be able to seamlessly transfer care between settings when needed. People will be empowered to manage their own health and to share in decision making.

SOCIAL INNOVATOR SERIES: WAYS TO DESIGN, DEVELOP AND GROW SOCIAL INNOVATION.
The Social Change Toolkit has been created to help you pull your ideas together in one place, and give you tips and guidance to help realise your community project.
It has been created for all changemakers, regardless of how big or small your social change ambition is.
Driven and funded by the Think Differently campaign team, the toolkit aims to make your life as easy as possible as you venture into social change.
Anyone can register for the ‘Your project’ section. This will give you access to fantastic online planning tools.
